Аня, спасибо за ответ - в общем, врач, как ни грустно, прав.:-( Неужели все так однозначно? Ведь резекция была в 1997 году? Почему же последствия проявились только сейчас? Мог ли золадекс их спровоцировать? Значит мне не стоит отдыхать, а надо срочно ехать на ЭКО? А если я не дождусь в ближайшее время месячных - у меня сегодня 27 день при моем обычном цикле в 24 дня до начала лечения.

Аня, а вот еще вопрос - а на сколько хорошей все-таки может быть реакция яичников через 10 дней после инъекции золадекса, учитывая, что это 3-й укол? Или все эти тяжелые побочные эффекты как раз и связаны с моими истощенными яичниками? Сколько времени у меня есть, как ты думаешь?

Как избежать генетического сбоя

В 1997г. после ЭКО была анэнцефалия плода. На 18 неделе сделали искусств.прерывание. В 1998г. родила ребенка. Сейчас думаем начинать заход по второму разу. Вопрос: как снизить вероятность генет.срыва (понятно, что амниосинтез нам уже прописан). Однако, профилактические меры возможны ли? Прием фолиевой кислоты, отказ от картофеля (якобы были сделаны выводы, что много случаев анэнцефалии связано с потреблением картофеля). Спасибо за ответ |

Нет, золадекс здесь не причем, просто это Ваш резерв или слишком сильная резекция. Месячных Вы скорее всего дождетесь, но лучше свяжитесь с врачом пораньше чтобы обсудить все детали.

Сколько времени в запасе - никто не ответит, но надо торопиться, это точно. Ответ в такой ситуации предсказать тоже сложно, но скорее всего он будет скудный, но я думаю не хуже, чем был бы без золадекса.
А такие побочные эффекты могут быть и при нормальных неистощенных яичниках. |

Здравствуйте, Лара! Про фолиевую кислоту Вы правильно написали, она действительно снижает риск аномалий развития головного мозга у плода. Ниже я привожу
тезисы двух статей на эту тему. Рекомендуемая американской академией педиаторов доза - 4 мг в день. Возможно у женщин группы риска (к которым Вы принадлежите) эту дозу стоит повысить (так заключают авторы второй статьи), т.к. у них в среднем фолиевая кислота хуже усваивается. Насчет картофеля я обнаружила довольно много статей, но все они датируются 70-ми годами. В них говорится о потенциальном тератогенном эффекте картофеля, пораженного некоторыми заболеваниями. Так что от него, я думаю, стоит воздержаться. Удачи Вам! |

Folic acid for the prevention of neural tube defects. American Academy of Pediatrics. Committee on Genetics. The American Academy of Pediatrics endorses the US Public Health Service (USPHS) recommendation that all women capable of becoming pregnant consume 400 microgram of folic acid daily to prevent neural tube defects (NTDs). Studies have demonstrated that periconceptional folic acid supplementation can prevent 50% or more of NTDs such as spina bifida and anencephaly. For women who have previously had an NTD-affected pregnancy, the Centers for Disease Control and Prevention (CDC) recommends increasing the intake of folic acid to 4000 microgram per day beginning at least 1 month before conception and continuing through the first trimester. Implementation of these recommendations is essential for the primary prevention of these serious and disabling birth defects. Because fewer than 1 in 3 women consume the amount of folic acid recommended by the USPHS, the Academy notes that the prevention of NTDs depends on an urgent and effective campaign to close this prevention gap. |

BACKGROUND: The Public Health Service of the United States recommends that all women capable of childbearing consume .4 mg (400 microg) folic acid per day to decrease the risk of having a pregnancy affected by a neural tube defect such as spina bifida or anencephaly. Three strategies are available to women to achieve this goal: use of dietary supplements; use of fortified foods; and/or increased intake of naturally occurring folate from foods. Identification of the most effective vehicle for delivery of folate to all women is critical in order to prevent these devastating congenital defects. OBJECTIVE: To investigate the difference in response to an oral load of folate both from naturally occurring food sources and synthetic supplements among women with prior pregnancies affected by neural tube defects and controls. METHODS: We compared the absorption of test doses of 400 microg pteroylglutamic acid (unconjugated or synthetic folic acid found in supplements) and 400 microg pteroylpolyglutamic acid (conjugated or food folate) in 10 women with a history of neural tube defect affected pregnancies and eight controls with normal birth outcomes. The folate test dose was given as either 32 fluid ounces of orange juice or a folic acid single supplement pill. All participants received each test dose at separate clinic visits. The response to each test dose was measured by constructing an area under the curve (AUC) from the serum folate levels at 1, 2 and 3 hours post dose and applying a t-test to compare within and between cases and controls. We also compared red cell folate, vitamin B12, zinc and homocysteine between cases and controls. RESULTS: Within group comparisons showed that the area under the curve was significantly greater for the pteroylglutamic acid dose compared to the pteroylpolyglutamic acid dose for both cases and controls (p=0.02 and p=0.03, respectively). In a between group comparison, control women had a greater serum folate response to both forms of the vitamin compared to the case women, but the difference reached statistical significance only for the pteroylglutamic acid dose (p=0.02). Other measured nutrients differed between cases and controls, but did not reach statistical significance. CONCLUSION: We conclude that for all women synthetic folic acid as supplements or fortified foods may be the best way to increase acute folate levels in the blood, and thus delivery to the developing embryo. Further, since case women had a diminished response to both forms of the vitamin, and some case women had almost no response, we speculate that women with prior affected pregnancies may need a larger dose of folate to elicit a plasma response equivalent to the general population. |

Уважаемая Аня! Объясните, пожалуйста, почему разные клиники переносят эмбрионы на разных стадиях развития? В одних "доращивают" до бластоцист, в других переносят пятиклеточные, в третьих - делают последовательно два переноса. Это связано с возможностями клиники (среды, оборудование), со сложившейся методикой- (отработан вариант- по нему и будем действовать) или по эмбриону "видно" доживет ли он до бластоцисты?
